2011-2012 School Year 4th Edition PPE Forms

In the March meeting of the TSSAA Board of Control, a decision was made regarding sports physicals, also known as "pre-participation exams" (PPEs). Beginning with the 2011-2012 school year, the board has decided to adopt the 4th edition PPE forms issued by the consensus of the American Academy of Family Physicians, American Academy of Pediatrics, and American College of Sports Medicine. This set of forms is widely acknowledged as a comprehensive and all-inclusive set of forms.

In the past, a physician's note from a prescription pad was considered valid for sports participation. With this change, the physician performing the physical must also fully identify himself with contact information and list any existing factors that could affect participation.

While the 4th edition PPE Form packet includes several pages, the emergency treatment/consent to participate and clearance form must be on file with the school to meet the TSSAA requirements. While it is beneficial for the school and its medical staff to have all pages on file, some patients may elect only to submit the emergency treatment and clearance form to maximize HIPAA compliance and patient confidentiality.
